This interactive 4-hour workshop is designed to highlight what goes into effectively planning and executing community needs assessments in a coalition model. Health Centers interested in partnering with Primary Care Associations (PCAs), health departments, hospitals, and community-based organizations on their needs assessments are encouraged to attend. Participants will gain insights from case studies including Maricopa County’s Synapse Hospital and Healthcare Partnership and other coalitions featuring Health Centers, hospitals, health departments, and more.
These real-world examples will be leveraged for small group activities to collectively identify community needs, create community health improvement plans, explore financial implications and resource savings, staff teams for continuity and integration, improve diverse community member buy-in, and drive systemic change through policy and advocacy.
