PFE1 - Involving Community Health Workers in Value-Based Care Arrangements for Better Health Outcomes

Session Description: SDOH have a profound impact on patient health outcomes particularly in the communities that health centers serve.Transportation, housing, and food insecurity are linked to health, yet current payment policies overlook the importance of addressing these issues. Community Health Workers (CHWs) are vital in bridging this gap as trusted members of their communities, connecting patients to critical services. By addressing SDOH, CHWs improve health outcomes and enhance the overall effectiveness of care delivery. Research has shown that CHWs can significantly improve the management of chronic conditions, expand access to preventive care, improve patient experiences, and reduce healthcare costs.

As healthcare systems increasingly move towards value-based care (VBC) arrangements, it is essential that payment policies evolve to reflect the critical role of CHWs. Despite their significant impact, the work of CHWs is often undervalued and undercompensated within existing payment models. Integrating CHW metrics into VBC arrangements not only acknowledges their contributions but also supports the sustainability of their positions, ensuring that this vital workforce can continue to thrive.

Health centers and the National Association of Community Health Centers (NACHC) should lead the charge in advocating for this change. They should work with the Centers for Medicare & Medicaid Services (CMS) and private payers to adapt value-based care models that reward the essential work of CHWs. By doing so, we can create a healthcare system that not only treats illness but also addresses the underlying social factors that contribute to health disparities, ultimately leading to better outcomes for all patients.
