PThD1 - Transforming the Health Center Value-Based Care Landscape: Connecting FQHCs to Participate in Value-Based Payment Models

Session Description: The Centers for Medicare and Medicaid Services (CMS) has set an ambitious goal of moving Medicare and Medicaid beneficiaries into value-based and accountable care arrangements by 2030. However, Federally Qualified Health Centers (FQHCs) have largely not participated in value-based contracts. As healthcare systems shift towards value-based payment models, FQHCs must navigate challenges to participate in Accountable Care Organizations (ACO) and preferred payor arrangements that address barriers to participation.

This presentation will highlight how Medicaid ACOs advance value-based care models for FQHCs. Community Care Cooperative (C3), the largest Medicaid ACO in Massachusetts and only Massachusetts Medicaid ACO composed exclusively of FQHCs, will share strategies to unite FQHCs for shared contracts, quality measures and services to improve patient outcomes, enhance care coordination, and manage healthcare costs. C3 will explore how it leverages data analytics, patient-centered care models, contracting frameworks, and strategic partnerships to advance FQHC Medicaid value-based care ACOs in Massachusetts and other states. DC Connected Care Network (DCCCN), an FQHC-led network in D.C., will then offer a real-world example of successful collaboration with managed care policymakers to drive quality improvements, operational efficiency, and financial sustainability.

Attendees, including FQHCs, primary care associations, and other stakeholders, will gain strategies for navigating governance, stakeholder engagement, strategic planning, procurement, data management, contracting, and policy alignment to support a transition to value-based care. The session will include a discussion on how to replicate C3's success, emphasizing the importance of strong partnerships and strategic alignment in achieving sustainable, high-quality care delivery.
